Mayo Clinic researchers have estimated the risk that pancreatic cysts with worrisome or high-risk features will progress to pancreatic cancer or advanced precancerous changes within three years, offering clinicians a more precise way to assess which patients may need surgery and which may be monitored more safely. The findings, published in Gastroenterology, could help reduce uncertainty for patients who are often told that imaging alone cannot fully predict how a cyst will behave over time.
The prospective study followed 230 people with pancreatic cysts that had worrisome or high-risk features. Researchers found that risk rose with the number of worrisome features: patients with one feature had about a 2% chance of developing pancreatic cancer or advanced precancerous changes within three years, compared with 4% for those with two features and 18% for those with three or more. The study also found that patients under careful surveillance were diagnosed at a very early stage, suggesting that monitoring may be appropriate for some higher-risk patients rather than immediate surgery.
Why the findings matter for early detection
Pancreatic cysts are commonly discovered during scans for unrelated reasons, but most do not become cancerous. The challenge for doctors is distinguishing cysts that can be watched from those that may become dangerous. According to the study, more precise risk estimates may help physicians and patients weigh the benefits of surveillance against the risks of unnecessary intervention, especially when a diagnosis is accompanied by anxiety and uncertainty.
Lead author Shounak Majumder said the goal is to identify what is likely to become cancer and what is not, so patients can be counseled more effectively. The study also offers a benchmark for evaluating blood tests and designing future research into early pancreatic cancer detection, including biomarker approaches such as CA 19-9. In a separate study, elevated CA 19-9 alone did not reliably identify which patients would develop cancer, reinforcing the need for better tools.

The researchers said the findings could support more personalized surveillance strategies and future studies of new biomarkers and early detection methods as part of Mayo Clinic’s Precure Research initiative. For patients and clinicians, the study adds a clearer framework for deciding when observation may be enough and when surgery should be considered sooner.
